Currently, the trend of the HIV epidemic is changes due to increasing the new cases among the young male population. Surprisingly, it is common among men who have sex with men (MSM) or known as homosexual. There are 3 problem statements in this study which are HIV infection trends, mode of transmission dominated by MSM and the level of awareness among the population especially in the east coast of Malaysia region. There are three research questions in this study which is to identify whether this population is aware of HIV, what is their level of awareness in practising sexual activity and how frequently HIV screening is done for them engaging in highrisk activities to ensure that they are free from any kind of virus. There are three research objectives, which are to measure the degree of HIV awareness, the degree of understanding and consciousness among high-risk populations and the prevalence and trend in HIV screening. The significance of the study for policymakers is the improved availability of sexual health education, the promotion of preventive measures, and the reduction of discrimination. In a public health perspective, allow to develop more effective treatments and preventives programs. Target for the community to promote inclusion, decrease stigma and good support system. The findings show that the level of knowledge toward HIV is medium sufficient but need for empowering comprehensive initiatives to ensure the misconception can be eliminated.
