Summary:Elevator safety is a critical concern in densely populated urban areas like Lembah Klang, where elevators are integral to daily activities. This study examines risk assessment practices and compliance with safety protocols during elevator maintenance at KONE Malaysia through the analysis of documented practices. Using a qualitative research design, it employs document analysis grounded in the Hazard Identification, Risk Assessment, and Risk Control (HIRARC) framework to assess the thoroughness of documented safety measures. The findings reveal notable inconsistencies in hazard identification across teams, with better outcomes observed in teams undergoing regular standardized training. Incident response records often lack detailed evaluations, underscoring the need for improved root cause analysis. Moreover, gaps in training and compliance are identified as key factors affecting the quality of risk assessments, highlighting the necessity of consistent training programs to enhance adherence to safety standards. This study contributes to improving elevator maintenance practices by recommending enhancements to training protocols and standardizing risk assessment documentation. Theoretically, it bridges the gap between the HIRARC framework and its practical applications in document analysis, offering a valuable reference for risk management practices. Future research is encouraged to adopt mixed methods, including interviews and observations, for a more comprehensive understanding of risk management in elevator maintenance.
