Factors causing incidents in a mega construction project: A qualitative study in Klang Valley

Malaysia being one of the developing country, understands the role of construction as one of the important and productive sector towards economy. Despite the growth, construction industry is categorized as a highly dangerous working environment due to the increasing rates of accident and fatality rates year after year. The aim of this study is to identify root cause of incident in construction industry. This study utilize the qualitative study which focus on approach based on the Ishikawa SM + 1 E method which stresses on several components which are divided into man, management, method, machine, material and environment based on incident report from 2016 to 2018 obtained from a construction of a mega project in Klang Valley. A total of 73 incident r , ports were obtained and analysis done to identify information such as the activity carried out and time when incident occurs, category depending on extent of injury or damage, type of incident depending on unsafe act or unsafe condition and the root cause of incident tb gain better understanding of the research topic. Results shows that the man factor are the most significant factor with method and environment factors also to be significant while the machine and material factors are slightly insignificant based on the data obtained. The study are very depended on the information obtained from the reports and some factors may be more significant compared to the other factors as this study are focus entirely on a mega project in Klang Valley. Thus, further research which involves a wider scale and obtaining data from the whole Malaysia is required to have a better picture of the actual scenario and root cause of the construction industry in Malaysia
